Angiotensin II
Vasopressor for septic shock; ICU use only.
What it is
Angiotensin II (Giapreza) is a synthetic version of the endogenous octapeptide angiotensin II, FDA-approved in 2017 for use as a vasopressor in adults with septic or other distributive shock. It is administered as a continuous IV infusion in intensive care settings.
Mechanism of action
Angiotensin II type 1 receptor activation produces potent vasoconstriction, increasing systemic vascular resistance and blood pressure. The mechanism is distinct from catecholamine vasopressors (norepinephrine, epinephrine) and provides an alternative or adjunctive vasopressor option for catecholamine-resistant shock.
Approved indications
Adult septic or other distributive shock requiring vasopressor support.
Why this is out of scope at The Tide
Hospital ICU medication only. Continuous IV vasopressors require intensive hemodynamic monitoring available only in intensive care settings. Not appropriate for outpatient peptide clinic.
