Selank and Semax are short peptide compounds used clinically for anxiety, cognitive support, and stress regulation. Both are administered as intranasal sprays and produce relatively rapid effects. Despite often being discussed together, they have meaningfully different profiles and serve different clinical purposes.
Selank is anxiolytic-leaning. It modulates GABA and serotonin pathways and is best known for reducing anxiety and stress reactivity without sedation. Patients describe a calm, clear-headed state — anxiety symptoms reduced, but without the cognitive dulling that benzodiazepines or other anxiolytics often produce.
Semax is cognitive- and focus-leaning. Patients describe improved focus, mental clarity, and stimulation without the jitteriness of caffeine or stronger stimulants.
Both peptides have decades of clinical research and use, though primarily outside the U.S. They’re prescribed off-label and prepared by licensed compounding pharmacies.

Selank is the better choice for patients whose primary concern is anxiety, stress reactivity, or sleep disruption tied to a busy mind. The effect is calming without sedating — most patients can use it during a workday without feeling slowed down. Useful for situational anxiety, ongoing stress states, and as an adjunct in patients with generalized anxiety symptoms.

Selank and Semax are useful tools for the right patient, but they don’t substitute for the basics of cognitive and emotional health. The patients who get the most from them have addressed the foundations first.

If sleep is broken, training is absent, or stress is unmanaged, no nootropic peptide is going to fix that. Patients who do well with Selank and Semax tend to have the basics in place.

Initial consultation
In person at our Houston clinic. We discuss your specific concerns — anxiety, focus, cognitive performance, or some combination — and what you’ve already tried. We’re honest about the evidence base and which peptide is the better fit for your situation. We also screen for conditions where these peptides aren’t appropriate.
Focused workup
Lab work to rule out competing causes — thyroid panel, B12 and folate, ferritin, vitamin D, A1c, and a comprehensive metabolic panel. Screening for sleep apnea or other treatable conditions that affect cognitive function. The goal is to make sure we’re not using a peptide to compensate for something that has a more direct fix.
Protocol design
Compounded intranasal Selank or Semax (or both) prepared by our partner pharmacies. Typical Selank dose is 250 mcg–500 mcg via intranasal spray, 1 to 3 times daily during periods of need. Typical Semax dose is 250 mcg–1000 mcg intranasal, usually morning and midday. Cycles vary — some patients use daily, others use intermittently during high-demand periods.
First prescription
Compounded nasal spray shipped from licensed compounding pharmacy. Detailed instructions on administration, timing, and what to expect. Effects are typically noticeable within 30 minutes of dosing, with cumulative effects developing over the first 2 to 4 weeks.
Reassessment
Honest review at 4 weeks. If the peptide is producing meaningful benefit, we continue. If response is partial, we may adjust dosing or timing. If response is absent, we have an honest conversation about whether to try the other peptide, adjust the broader approach, or acknowledge these aren’t the right tools for your situation.

How much does treatment cost?
Compounded Selank or Semax typically runs $150 to $300 per month depending on dosing, frequency, and quantity. Patients using both peptides typically pay $250 to $450 per month combined. Our clinical service fees cover physician oversight, lab review, and follow-up separately. We’ll discuss specifics during consultation.
What’s the evidence base?
Both peptides have decades of clinical research, primarily from Russian and Eastern European institutions, where they’ve been studied and used clinically. The randomized controlled trial data from major Western journals is thinner. The clinical experience and animal-model data are substantial. We discuss this honestly with every patient — the evidence base isn’t at the level of mainstream pharmaceuticals, but the safety profile is favorable and the mechanism-based rationale is reasonable.
Are they FDA-approved?
No. Both are prescribed off-label in the United States and prepared by licensed compounding pharmacies. Off-label and compounded prescribing is a standard part of medical practice. We discuss the regulatory context openly during consultation.
What are the side effects?
Both peptides are well-tolerated. The most common reported side effects are mild — occasional nasal irritation from the spray, transient headache, or rarely, mild sedation (Selank) or stimulation (Semax) beyond the intended effect. Serious side effects are uncommon. The peptides don’t appear to produce dependency or withdrawal — you can stop using them without issue.
Can they replace my anxiety medication?
Generally no, and we don’t approach Selank as a replacement for established anxiety medications (SSRIs, benzodiazepines, etc.). For mild situational anxiety or stress reactivity, Selank may be useful on its own. For moderate-to-severe anxiety disorders, Selank is more appropriately considered as an adjunct alongside mainstream treatment, in coordination with your prescribing clinician. We discuss the context honestly during consultation.
Will Semax help with ADHD?
Some patients with ADHD report improvement on Semax, but it isn’t a substitute for proper ADHD evaluation and treatment. If you suspect ADHD, the right path is a complete diagnostic workup with a psychiatrist or behavioral health specialist. Semax may be useful adjunctively for cognitive performance once that diagnostic foundation is in place. We don’t prescribe Semax as a primary ADHD treatment.
Can I use them daily long-term?
Most patients use them episodically rather than continuously — Semax during high-cognitive-demand periods, Selank during stressful times. Daily long-term use is less well-characterized in the available research. We typically design protocols with built-in cycles (e.g., use during workday, take weekends off; or use for 4 to 6 weeks, then a brief break). Long-term effects of continuous daily use aren’t fully established.
